Citizen Questionnaire Form

A requirement for the update of the Madison County Multi-Jurisdictional All Hazards Mitigation Plan is to have residents in Madison County to fill out an Questionnaire.  You can help protect lives and property from natural hazard events in the County by taking a few moments to complete this questionnaire.

All Questionnaires must be received by January 28, 2020.

Citizen Questionnaire Button below.  To fill out the questionnaire you can print it out, fill it out and drop it off at the Village Hall at 6810 Godfrey Road, Godfrey, IL 62035 at the Building & Zoning Window office or the Clerk Office.  You can also mail it to the Village of Godfrey, 6810 Godfrey Road, Godfrey, IL 62035, Attention: Engineer or to the Madison County Emergency Management Agency, 101 E. Edwardsville Road, Wood River, Illinois 62095.  Depending on your Adobe Reader, you may have to “downloading” the file or “open with a different viewer” the Questionnaire when it comes up on your screen to have an editable form.  If you get an editable, please fill out the form and hit the submit form button on the bottom of the questionnaire.  Hopefully the submit form button will bring up your e-mail account on your computer in which to send the questionnaire to the County’s contracted firm that is overseeing the work on this Hazards Mitigation Plan update.  If not, print the completed form and e-mail it to abostwick@aecspfld.com make subject: “Form Returned: Citizen Questionnaire – Madison County”
